Patient Advocate – Social Worker

The Patient Advocate’s primary responsibility is developing in-market tactics to streamline patients’ paths to appropriate Medicated Assisted Treatment (MAT) plans with axialHealthcare’s new Opioid Use Disorder (OUD) offering. The Patient Advocate will partner with external stakeholders to better understand the treatment workflow and develop opportunities to reach patients earlier on in the treatment process. Our ideal candidate has a strong healthcare and/or behavioral health background, and the ability to show empathy, patience and a vested interest in creating a welcoming environment for patients suffering from OUD. The Patient Advocate will work cross-functionally with teams across the organization to deliver clarity, alignment and focus to axialHealthcare’s OUD solution.
Specific Duties:
  • Develop the in-market tactics to move patients with OUD into active MAT treatment
  • Efficiently initiate and handle inquiries from physicians for patients referred to axialHealthcare
  • Complete admissions process, including but not limited to: conducting intake screenings to assess the client’s situation and explaining the steps to recovery
  • Effectively follow-up with patients in need of MAT treatment but who have not yet opted-in, at specified time intervals
  • Exercise effective use of soft skills, including but not limited to: active listening, questioning techniques, building rapport, voice and paraverbal communication, empathy and motivational interviewing
  • Facilitate video screenings with addictionologist while with patient  to determine clinical appropriateness and correct placement for MAT treatment
  • Partner with external stakeholders to understand the various ways axialHealthcare fits into the treatment workflow
  • Understand, identify and develop opportunities to get to the patient faster
  • Create a niche within the hospital that creates value for the providers by taking administrative work off their plate
  • Assist in developing content that helps the patient stay connected while providing value to axialHealthcare
  • Understand what motivates patients to keep coming back and neutralize the things that do not


  • Bachelor’s degree or equivalent educational or professional experience and/or qualifications
  • 2 to 3  years experience in addiction medicine or hospital setting
  • Empathy, patience and an unyielding interest in creating a welcoming environment for patients
  • An unwillingness to accept failure as the final outcome but an understanding that we will fail – addiction is a disease and we will not cure everyone the first time, but we will keep trying
  • Equivalent experience includes one at least one of the following: use of a CRM system (preferably Salesforce),  direct patient contact via phone or face-to-face, etc.
  • Computer proficiency: strong typing skills and the ability to multi-task (type and talk) is required
  • Strong healthcare / behavioral health (i.e. psychology, nursing, social work, etc.) knowledge and skills preferred, Drug and Alcohol Counselors or Mobile Assessors a plus
  • Ability to read and interpret written information; write clearly and informatively; ability to speak clearly and persuasively in positive or negative situations; listen and get clarification; respond well to questions; demonstrable group presentation and customer service skills
  • Strong goal achievement history with the ability to strategize, persuade, overcome objections and negotiate in a fast pace environment
  • Ability to multi-task and achieve metric goals at a high level with or without direct supervision
  • Must be a licensed social worker (LSW) 
  • Excellent problem solving and decision-making skills
  • If in personal recovery, a minimum of 2 years clean and sober
  • Must be located in Birmingham Metro area.
